Questionnaires to monitor your progress
We might ask you to fill in questionnaires about your health and quality of life, before and after your treatment. These are sometimes called patient reported outcome measures (PROMs).
Your feedback helps us measure the outcome of your treatment at different times.
We encourage you to take part, but if you do not want to, it will not affect your care.
Benefits of these questionnaires
They can help your clinical team understand how you feel about your health, and which parts are bothering you. The information you give can help guide decisions about your care and treatment. It can make the time you spend with your clinician more valuable, allowing them to focus on what matters to you the most.
What sort of questions are asked
The questionnaires ask about your health. They cover many areas including your current symptoms, as well as general wellbeing. They can let your clinicians know how you are feeling and functioning, from your point of view.
How the results will be used
Your responses will be reviewed and discussed at the appointment with your healthcare team. This will help to monitor your progress and address any symptoms you are having.
Completing questionnaires in MyChart
MyChart is an electronic platform that keeps you connected to your healthcare team. Find out how you can create a MyChart account.
Once you have an account, you will be asked to complete the questionnaire at the start and end of treatment, and before each follow-up appointment. Some patients might be asked to complete one at other times during their treatment too.
To make sure you’re notified promptly when a questionnaire is ready to be completed, please enable push notifications in MyChart.
If you do not receive an activation code for MyChart sign up, please speak to your healthcare team.
How long a questionnaire takes
Each questionnaire should take less than 10 minutes to complete. The first time might take longer as you familiarise yourself with the questions.
Your usual care
Your responses are used alongside your usual communication with your team. The questionnaire will only be reviewed during your consultations.
If you have symptoms which need urgent attention, it is important that you contact your team in the usual way for support.
Choosing not to participate
If you do not want to fill out the questionnaires, you can opt out at any time, and it will not affect your care. Your appointments will still take place as usual, but the doctors might have to ask you more questions to understand how you are feeling.
How your information will be stored
Information from these questionnaires will be stored securely in your electronic health record. We will store your information safely, and we have strict rules about how it can be used.
