Book your maternity care with us

Complete the online antenatal self-referral form to refer yourself to King’s College Hospital. You do not have to see your GP before contacting us.

If you have a MyChart account with us, please use the sign in option on the form to complete the referral. If you do not have a MyChart account, you can continue to self-refer without one and create your account in the process.

Please fill out as much of the form as possible and provide your correct contact details. It takes 7 working days to process your form, and you will receive your booking and scan appointment date afterwards.

You will not receive a confirmation that we have received the referral but if you are not contacted with appointment details within 7 working days then please contact the team by email [email protected] or call 020 3299 8131.

We see pregnant women and people who live in the below postcode areas in Lambeth, Southwark, and Lewisham. If you live outside this catchment area, we will also consider your referral:

• SW2, SW4, SW8, SW9, SW16
• SE1, SE4, SE5, SE11, SE14, SE15, SE16, SE17, SE19, SE21, SE22, SE23, SE24, SE25, SE26, SE27
• CR7

Referrals from GPs

GPs can also refer to all clinics using the Antenatal GP and medical referral form.

For community bookings where a nuchal scan is required, please submit the form and state which community midwifery team is providing care.
