Urgent suspected cancer referrals

To refer a patient with suspected cancer (previously known as two week wait referrals), GPs and primary care referrers can download and complete a suspected cancer referral form from the Transformation Partners in Health and Care website.

Please then refer via NHS e-Referral where possible (the preferred method). Include the patient’s correct contact details on the referral, including both a mobile and landline phone number, where available.

Informing suspected cancer patients

Let patients know you are making a suspected cancer referral, and that we will contact them to arrange an outpatient appointment, which will take place within two weeks.
