New Outreach Therapy service support King’s patients

16 December 2025 - A new therapy outreach service has been launched across South London to provide support for King’s College Hospital patients, helping to deliver safe and timely discharges for patients who no longer require hospital treatment.

The King’s Outreach Therapy Service, a team of occupational therapists, physiotherapists and a rehabilitation assistant, is an innovative new service helping make sure that newly-discharged patients are able to manage independently at home in a safe environment.

Michelle Mote, Clinical Lead Occupational Therapist, at King’s, explained: “Over recent years, more and more patients have needed support from community therapy services following inpatient admissions, and we decided to pilot a new service to provide patients with the support they needed in the familiar environment of their own home.

“Through the King’s Outreach Therapy Service, once patients are discharged from hospital, our team can step in to help bridge the gap between hospital and home. Our response time depends on each person’s needs, but our team can be with patients within 24 hours of the referral.  We can carry out a range of different interventions in the community, helping people feel safe in their home after an acute hospital admission, providing equipment to ensure people can get around their home safely, as well as education for caregivers, and putting people in touch with community services such as memory clinics and befriending services.

“We’re extremely proud to say that this is now a permanent and well-used service, which has improved discharge times and prevented unnecessary hospital admissions. Our team has helped over 500 patients across South London, with 100% patient satisfaction reported, and we are looking forward to helping even more people return to the comfort of their homes quickly.”
