Cardiac health and rehabilitation
What we do
The multi-professional team at King’s Cardiac Health and Rehabilitation service includes specialist nurses, a physiotherapist, a psychologist and an exercise specialist.
We ensure our patients have the highest quality of care. We help them to understand and manage their heart condition, so they can improve the health of their heart as well as their quality of life and general well-being
We provide patients, their families and carers with individual advice, support and information. We offer a full rehabilitation service for local patients, including a home visit, clinic appointments, psychological support, help with changing their lifestyle and a cardiac health programme of exercise, education and relaxation.
We send patients a questionnaire and other paperwork for them to complete before their clinic appointments, which they should bring with them when they attend. In addition, they should bring with them an up-to-date list of medications and any other paperwork relevant to their heart condition.
Our advice line for patients is available Monday - Friday, 9am - 5pm (24-hour answerphone).

Preparing for your appointment

Conditions we treat
We treat/provide the following treatments:
- myocardial infarction
- cardiac surgery (coronary artery bypass or valve surgery)
- angioplasty/stent
- heart failure patients referred from hospital heart failure or community heart failure team
- other cardiac patients/out-of-area patients on individual assessment.
What is required before referring a patient
Patients should bring an up-to-date list of medications and any other paperwork relevant to their cardiac event/intervention to their clinic appointments.
Booking a patient at King's
Routine Referrals
Patients are referred to our service:
- as inpatients at King's
- from other local hospitals
- by their GP if they have had a cardiac event or intervention within the last six months.
For referrals outside King's, please send information and a covering letter with patient details to:
